Case Conferencing
Case conferencing is a routine, centralized process that helps community leaders and housing navigators monitor and advance the progress of various people toward housing. Our CES team works in tandem with case workers and community leaders to ensure that we as a community are tracking, engaging, and connecting all persons experiencing homelessness to permanent housing resources and supports.
Goals of Case Conferencing
To ensure holistic, coordinated, and integrated assistance across providers for all persons experiencing homelessness in the community;
To review progress and barriers related to each individuals housing goal;
To identify and track systemic barriers and strategize solutions across multiple providers;
To clarify roles and responsibilities and reduce duplication of services.
Monthly Schedule of Case Conferencing
North Shore, Windward, Waianae Case Conferencing: 1st Wednesday of the month from 1pm - 2:30pm
Youth Case Conferencing: 2nd and 4th Wednesdays of the month at 10am - 11:30am
Veteran's Case Conferencing: 2nd and 4th Wednesdays of the month from 1pm- 2:30pm
Honolulu Case Conferencing- 3rd Wednesday of the month from 1pm - 2:30pm
